All applicable payments (co-pays, etc.) are expected when services are rendered. We will file claims for office services and surgical procedures. We participate with most insurances, however, it is the responsibility of the patient to secure any referrals needed in order to comply with insurance guidelines. Any necessary referrals need to be brought with the patient on the day of the office visit. The patient is ultimately responsible for any amounts not paid for by the insurance company. Payment may be made by cash or check. We also accept Mastercard and Visa.
